Unattached Patient Clinic

UPDATE

For the past 18 months, it has been a privilege to care for members of our community through the Unattached Patient Clinic. Thank you for trusting our team with your healthcare needs, it has been an honour to provide you with our care and support. 

The Unattached Patient Clinic (UPC) located at 565 Sanatorium Road is officially closed effective March 31, 2026. 

The clinic was created as a temporary program to help community members without a family doctor or nurse practitioner access primary care services until they can find a permanent primary care provider. Although the clinic is now closed, our focus remains in transitioning and supporting our patients to connect them with a permanent healthcare provider. 

We encourage all our Unattached Patient Clinic patients looking for a family physician or nurse practitioner to register with Health Care Connect, Ontario’s free, centralized program that helps people find ongoing healthcare. Many patients are successfully matched with a provider, and our team works closely with the program to support access to long-term healthcare.

If you prefer to register by phone or require assistance, please call 1-888-579-6707, Monday to Friday between 8:30 a.m. and 4:30 p.m., or email: HealthCareConnect@ontariohealthathome.ca. 

During this transition, our clinic team continues to contact patients to help connect them with a permanent family physician at Escarpment Health Centre or other local clinics. Anyone who has not yet been contacted is asked to please call 289-309-4488. 

We understand that changes in healthcare services can feel uncertain. Please know that support is available, and every effort is being made to help patients stay connected to care.
