Book an Appointment for the Unattached Patient Clinic

This page is for ‘unattached patients’ to schedule an IN-PERSON appointment in the Unattached Patient Clinic. This clinic provides medically necessary care services to you while you are looking for a family doctor. No video or phone appointments. 

*If you already have a family doctor, primary care provider or need urgent care, please DO NOT book an Unattached Patient Clinic appointment. Please attend your designated after-hours or urgent care clinic.

Important Note:

The Unattached Patient Clinic provides medically necessary health assessments, chronic condition care, preventative health visit and cancer screenings.

Our physicians do NOT prescribe narcotic, sleeping pills, or stimulants. Please contact Health Care Connect 1-800-445-1822 for further assistance or contact Ontario Health811. We also are unable to guarantee access to a specific provider, gender or language service (please bring an interpreter if required) in our Unattached Patient Clinic. 

Our doctors are NOT accepting patients into their practice. Please do not book an appointment for the purpose of looking for a family doctor with the UPC. You may refer to the Hamilton Family Health Team website to see a list of family physicians accepting new patients here.

Preparation -  If it’s your first time visiting the clinic, please arrive 15 min earlier to fill out our patient registration form. 

Please bring and present the physical copy of your valid Ontario/Provincial Health Card each time you visit our clinic. A photo or photocopy of the card will not be accepted. Expired or invalid health card will not be accepted. If you have a temporary health card document, please bring the physical paper of the document, AND the physical copy of your expired health card/photo ID indicating your date of birth.

Cancellation - If you need to cancel or change your appointment, please do so at least 24 hours before your appointment time. Cancellation without 24 hour notice may be subjected to a no show fee.
