Dear Patients,

We write you this letter to inform you of the re-location of your current family practice to the newly established Escarpment Health Centre at 565 Sanatorium Road on Hamilton’s West Mountain. Together, these seven family physicians and their teams, are forming a new primary health care ‘hub’ with the aim of better serving patients through interdisciplinary care and community engagement. Please note that your family doctor is not changing nor will you be required to ‘sign up’ at Escarpment Health Centre. Your personal health record will remain the same with no effect.

We recognize that, as patients, you become accustomed to seeking care in a particular and familiar location and that these sorts of transitions can be an adjustment. We ask for patience and optimism as the creation of a larger and more integrated health centre enables the delivery of a greater number of health services on-site. As part of the Hamilton Family Health Team, Escarpment Health Centre will continue to be inherently interdisciplinary with its mental health counselling, clinical pharmacy, dietician services and consultation psychiatry. There will also be health profession learners from the Department of Family Medicine at McMaster University and the Bloomberg School of Nursing at the University of Toronto.

We anticipate seeing patients on-site at Escarpment Health Centre by June 2023. Until then, we will continue to service our patients out of our current office locations. Patients can be kept up to date by visiting our website or contacting their respective family doctor’s office for more details. Escarpment Health Centre offers free parking, or for those taking public transit, we are accessible via HSR bus route 33 (click here for bus route information). Please note that there will be active construction on-site for some-time as we finalize our new, permanent home!
